The below links will allow you to download the most commonly requested forms. 

If you are downloading a Voluntary Statement form to provide a sworn written statement, it will not automatically generate a complaint.  You must file your complaint with the police department by either calling us or in-person.  A written statement must be signed in the presence of a Notary Public. Several of our personnel are Notary Publics, but notaries can also be found in our Town Clerk's office and banking institutions.  We suggest that you bring your written voluntary statement to the police department before signing it to have an officer review it to ensure the information we need to pursue your complaint is in detail and complete. Arrangments will then be made for your signature and swearing to the truth of the statement in front of a notary.
